Please use the form below to request an angel.  Be sure to complete all the requested information and please double check the mailing address before submitting the request. Include any notes or information that you may wish to share that might help guide our angel artists as to what type of angel is needed.

Whenever possible, please supply a HOME address rather than a hospital.  Our experience has been that by the time our angels arrive at the hospital, the recipient has already been discharged and our angels are returned as 'undeliverable'.

We are very careful with the names and mailing addresses that we are given for this program.  Many people, especially the elderly, are very uncomfortable with their personal information being given out over the Internet. That is why very special care is taken with each request we receive.
